DSE Self-Assessment Form Template
A display screen equipment (DSE) self-assessment the user completes about their own workstation, checking the screen, keyboard and mouse, chair and posture, desk and environment, with space to flag any problems and record the actions taken. It comes with a short guidance document on good workstation set-up.
The Health and Safety (Display Screen Equipment) Regulations 1992 require employers to assess DSE workstations and reduce the risks. This self-assessment gives you a practical way to meet that duty for office, home and hybrid workers and supports the risk assessment requirements of ISO 45001.
